Fundamentals
The Planes of Existence are designed to help humanity understand the workings of not only the physical existence, but the metaphysical entities that correspond with our bodies that produces life and helps maintain it. Every Planes of Existence composes the typical living being's essence in one way or another through a form of corresponding. Only certain Planes can freely correspond with other planes without either becoming annihilated by the Plane in which it seeks to join, or not influencing it at all. The main base Planes are: The Physical Plane (1), The Spiritual Planes (3), The Mental Planes (3), Inner and Outer Planes (Infinite).
Each Planes is separated by a form of a void which doesn't allow the energy from other Planes to intersect or conflict with the opposing entity. But, under certain manipulations or magics, such accomplishments are commonly done through drastic measures uncommonly perceived. A lot of things that players do can be considered in one way, shape or form, a type of Planar Manipulation. (ex: Psionics, magic, etc.)
The descriptions here are not absolute rules, they are designed to give a basis for interacting with such ideas in RP.
The Planes
The Prime Material Plane
The Prime Material Plane is the most common and comprehendible Plane, considering it holds all the physics known to man. It's where all physical objects are, which persists of one's body, their attire, weapons, the ground they stand on, the food they eat, where they sleep, etc. Everything we humans see on the Prime Material Plane. This is typically the base Plane for everything since it holds the most logic, yet it also the most common plane in which is interfered by coexisting Planes.
The Prime Material Plane can be seen as a place where the energies of the other planes converge to form something of a neutral state. Whereas planes such as the elemental plane of fire or water are predominantly of one element or another, the Prime Material Plane holds these two forces in roughly equal amounts. The same can be said from the specific energies of most every other plane. Disruption from the other planes, usually through an intentional connection, can cause this balance to shift in the favor of one or another form of energy.
The Spiritual Planes / Transitive Planes
The Spiritual Planes are pretty much given away by the name. In this case, it deals with all the spiritual entities that are part of existence. The soul, in this case, is commonly influenced in all of these. In most theories, the Ethereal and Astral are either switched, or hold similar qualities, but in my theory, it works quite well.
The Phantom Plane (The Spirit Plane)
This is the plane where our spirit resides. It gives us hope of obtaining an after-life. If there is no soul, there is no hope of going to the Astral Plane to cross over into the after-life itself. It holds no real value when you're alive exactly.
The Shadow Plane
The Shadow Plane is rather an unnecessary Plane in existence, but does give logic to shadow creatures and like-things that steal souls, and go into some odd land of the shadows. No souls under normal conditions exist upon this plane, but creatures that aren’t Astral entities that take a need for soul reaving, reside upon this plane. The shadows that exist upon the Prime Material Plane are not a gateway to the Shadow Plane, as that’s rather illogical. Shadows on the Prime Material Plane are an object obstructing the path of light to a patch of land which is dimmer compared to its surrounding atmosphere. A physical form can exist between the Shadow Plane and the Prime Material Plane, but not everyone can. If someone were to get tossed into it, no real harm would be done compared to something like being tossed into other ones. The Shadow Plane is thought to be similar to the Prime Material Plane, just colors and shapes are disoriented and faded. Stealing souls on the Shadow Plane is different then other formats. When stealing a soul upon the Shadow Plane, you somewhat create a bridge to either the Prime Material Plane, or one of the other Spiritual Planes, so a target can be locked upon. This bridge is a necessity due to the simple fact that souls aren’t seen upon it, but Shadow entities can influence it. In example, someone creates a bridge to the Prime Material Plane. A shadowlike figure which represents your figure would appear upon it. On the Prime Material Plane, considering you’re not exactly stepping onto it, you’re considered intangible. You can move forth and not even touch their physical form, yet rather their soul. Thus, reaped.
The Shadow Plane isn't the black thing that follows our bodies around. You can't jump into a shadow and magically appear in the Shadow Plane. The Shadow Plane is the first layer of the Transitive Planes, meaning, it deals primarily with the boundaries of our soul. Some soul-stealers are shadows and such, as all this world really deals with is the exact look of the Prime Material Plane, except the colors are distorted, there are barely any living creatures naturally roaming around and all of that good stuff. The Shadow Plane is primarily known and used for to travel about existence, to get to other places quicker and such.
The Ethereal Plane
The Ethereal Plane is the plane of the living soul. It's composed of spiritual entities, and is separated from its coexisting Spiritual Planes, along with the other Planes of Existence. Souls, corresponding to a beings' essence, shift along this plane along with all the other being's soul. Not many special properties apply in this case.
This plane is the second layer of the Transitive Planes, as it's often viewed at as a sea of life. A never ending milky sea of drifting souls, corresponding to the essence of their creator. This is the plane where living souls are harvested, and merely roam about. That's why our souls and bodies do not exactly mix, as they're on separate levels of existences, which compose existence itself.
The Astral Plane
The Astral Plane is the plane of the dead soul. This is where Wraiths exist and roam, taking the living souls to the after life via the River of Styx which exist between the Astral Plane and the Outer Planes. When a soul, or anything else composed of living entities enters this plane, they're consumed with Astral entities, and typical deemed dead, awaiting passage to the afterlife. If you take a soul and put it into this place, its living and sentient qualities will become no more, and thus become dead and decaying.
This is the third layer of the Transitive Planes, as the Astral and the Ethereal are seemingly constantly debated over. What I view the Astral to be is a simple place of nothingness. Think of it as a never ending white room of silence. Some wraiths, githyankis and such roam around, but they're the creatures that naturally exist upon this plane. This plane leads to the outer plane, which is the after-life, as it connects to the River of Styx, which leads to the outer planes. If someone were to enter this plane, they would be over-whelmed with death, and utterly dead.
The Mental Planes
The Surreal Plane
The Surreal Plane is where the conscious mind exists. Your mental sentience and prowess primarily exists upon this plane, and doesn’t leave its form until an attempt to do so is conducted; such as extreme mental activity such as psionics. Primarily, this is your kinetic brain activity that just perpetually revolves in a circuit as it travels through out a being’s body. The Plane itself doesn’t hold any special properties on its own besides the elementary components.
The Subtle Plane
This is the third of the mental planes. This is what seperates us from the next sucker in life. This is where our free will, desire, personality, and all that other emo stuff resides. It influences the Surreal Plane like a "God" over its people.
The Subtle Plane is more along the lines of a Plane which is known by few, besides those who have either read my theory previously, or did some philosophical work with Veazey. This is the Plane where the subconscious exists. It’s separate from the normal conscious due to the simple fact that the subconscious navigates vital components of the body, such as breathing, beating heart, emotion, etc. One’s free will, personality, and very mental structure exists here. If one where to openly become influenced upon the Subtle Plane, one could change the target’s personality, enslave their minds, and utterly dominate them upon the entire mental scale. Considering this plane is such a sacred place, sacred as in vital, it harder to come into contact. I consider the Subtle Plane to vaguely be intertwined through the Surreal, as it’s like tiny membranes on a planar level. Inside one’s Surreal existence, deep with in it, is one’s own Subtle Existence. So, obtaining such contact would be more difficult, but the sly manipulator could easily find loop holes.
The Cerebral Plane
The Cerebral Plane is a Plane that I would find to be a necessity for all theories that revolve around the most “magical” side of logic. (Even though things such as magic is the defiance of scientifical logic, it still holds boundaries of its own logic in which its own laws allow what can potential occur and not occur, as well as a set of rules to which what could happen when something is done.) The Cerebral Plane is the plane in which mental waves descend from their proper Surreal origin, and shift through out the Cerebral Plane. The Cerebral Plane is a mental plane which can influence all the other planes in a form of influence manipulation. As all would know, mental entities can’t exactly exist upon any other plane without either becoming consumed by it, or rather joining it, turning something transmundane and intangible, to something physical. Consider the idea of the Cerebral Plane to function more along the lines of a vein system that roots through out the Planes of Existence, as the walls of the veins are similar to paper, and the mental power influences the entities which either composes the mass, or whatever one is trying to manipulate.

The Inner Planes
The Inner Planes help explain what occurs when magic is conjured through the elements. As one would know, magic is the defiance of logic, but there is a logical explanation. There's a separate level on the inner planes that holds each element. (EX: Fire, Water, positive, negative, etc.) When it's magically conjured, it influences the destined plane, and creates an imprint on it. This imprint doesn't feed off of the Prime Material Plane, yet rather the magic energy put into keeping the imprint upon the Prime Material Plane, or whatever destined plane it is.

Each plane has elementals that exist there, and each plane has abundant amounts of its energy type. Most planes are harmful to travel on as the extremes of their energy can often destroy a being. Simple.
The Outer Planes
The Outer Planes are rather simple, the Planes of the afterlife. The Seven Heavens, the Nine Hells, or whatever. It's filled with negative entities of death, and when put into contact with life, negation occurs.
The outer planes are connected to existence through the Astral Plane, it's the last piece that makes up existence, and it located in the outer shell, or layer itself. It's where the Seven Heavens, Nine Hells, Demi-Planes, and etc are located.
Other Planes
Other Planes are rather unnecessary, considering they're either created by someone else, or sketchy when it comes to its composition and purpose. Created Planes, such as the Transmundane and the Phantasmic Planes are created to fit their creator's needs, rather them actually compose existence itself. Such Planes as the Temporal Plane, which is believed to be the Plane of Time, is rather sketchy, considering how a sole plane would influence every plane is vague. Rather, the thought-of 4th Dimension would make more logic. As I've said, this is my Planar Theory, and this is how I use it. There will always be conflict and variation, as the point of mastery is not knowing definitions, or annihilation of foes, but how to adapt to these conflictions and pull out victory in the end.
The Temporal Plane (Time)
This Plane is kind of an off-limits plane, considering most of the people like myself view manipulation of time itself to be god-modding, but I'll explain it to you guys just in case if you don't believe that, and/or someone tries to throw some time manipulation crap around at you. Basically, this is where time resides. You could say it intertwines with in the dimensions making the fourth dimension, as well as all the planes itself. It's like it exists, then again it doesn't. No one force in a plane can extend their hand out, and expect to touch time, it takes a greater force.
Someone like a Time Mancer and such, none the less one who is composed of Temporal entities. But in reality, everyone is composed of it since we're all effected by it, but can't just reach out and push it away from anything. It takes great manipulation and concentration.
Demi-Planes
Basically, these planes are created by certain people that need it to make their character function and such, but it's only temporary. Once the creator dies, so does the plane itself. It has its own laws and principles.

IRL, yes. So IC that would make it the ability to manipulate certain things in material existance. Like, if a guy with a sword decides to charge me, I could manipulate the materia of the blade (the steel or metal) and bend it to my will, essentially making it harmless...But that's probably far off.

There would be no gain in it since right when you bring it back to the corporeal, it'll transport back to the Phantom since it's not consisted of any physical traits. The only thing you can do with it is either trap it in a prison of sorts or destroy it. Just like trapping a finger in a bottle, you need a prison that corresponds to spirit energy, aka phantom entities.

If you did what Erasure suggested and brought the Spirit through to the Corporeal, and once it was there, induced it with corporeal entities, would it be efficiently 'trapped'?

When on this plane, and if you don't protect your corporeal medium, you're basically consumed by the Astral and annihilated and thrown into the Styx River, which floats you down to the Outer Plane (AKA the After-Life)

Most people don't think about defending this place, and don't worry, I already have my ways of taking over the mind. Basically, all you need to do is induce what you want, which is also Subtle, into their Subtle entity, and if all is successful, you can influence their decisions and maybe even take over by creating a direct path to this plane.

Such Planes as the Temporal Plane, which is believed to be the Plane of Time, is rather sketchy, considering how a sole plane would influence every plane is vague. Rather, the thought-of 4th Dimension would make more logic. As I've said, this is my Planar Theory, and this is how I use it. There will always be conflict and variation, as the point of mastery is not knowing definitions, or annihilation of foes, but how to adapt to these conflictions and pull out victory in the end.
The Temporal Plane (Time)
This Plane is kind of an off-limits plane, considering most of the people like myself view manipulation of time itself to be god-modding, but I'll explain it to you guys just in case if you don't believe that, and/or someone tries to throw some time manipulation crap around at you. Basically, this is where time resides. You could say it intertwines with in the dimensions making the fourth dimension, as well as all the planes itself. It's like it exists, then again it doesn't. No one force in a plane can extend their hand out, and expect to touch time, it takes a greater force.
Someone like a Time Mancer and such, none the less one who is composed of Temporal entities. But in reality, everyone is composed of it since we're all effected by it, but can't just reach out and push it away from anything. It takes great manipulation and concentration.

A long time ago, when I very first met Tenishna on CC, he and I fought. He didn't mention the Temporal Plane, but instead regarded time in streams and rifts. Time can exist anomalitically through rifts and streams according to that long ago fight. Say one with enough omnipotence, (a Seraph for example. ^_~), extends their will unto this plane. The Temporal. Could they force it into an anomalytical state within the Prime and force a person to exist in a different state of time within a rift and/or alternate stream? Also, Mundus(noobish but still), regarded time in streams, and suggested that time exists with separate and more infinite amounts of energy as the streams increase. Could said Omnipotent being ascend through the streams of time to gather energy? Just asking random questions. xD

Everyone has their theories when it comes to time, but the very first battle between Token and Levistus, Levistus ascended himself to a higher time-stream to basically heighten his bodily functions and such. Anything is possibly really when it comes to manipulating time.

Demi-Planes
Basically, these planes are created by certain people that need it to make their character function and such, but it's only temporary. Once the creator dies, so does the plane itself. It has its own laws and principles.

Is it possible for an entity to be made from(or by) demi-planes? Not that Loki shall be used anymore, but I should at least know if what I was doing was possible.
